Here are the steps I took to get the 100% success rate for the Mina cosmic upgrade. Just got to choose one of the three cards demanded by the power shrine and match it perfectly. There was also a step to get the first card using the health shrine and getting the card I wanted to upgrade to Quality 10 first. Since using the power shrine doesn't use up a shrine use I will sometimes store a charge when I am at 0 shrine uses.
